Insurance
Coverage Many
insurance plans, including Medicare offer coverage for prosthetic
services. Horizon Prosthetics accepts
Medicare and many other insurance plans.
All insurance plans offer different coverage for prosthetic services
and Horizon Prosthetics will verify benefits prior to providing
services. Horizon Prosthetics
participates with a third-party company to contract with many private
insurance plans. Mandy will discuss
this third-party billing system at the time of your appointment. It is
important to bring your insurance cards at the time of your first appointment
or anytime you change your insurance coverage or policy. A photo copy of your insurance cards, front
and back will be made at the time of your appointment. It is your responsibility to notify Horizon
Prosthetics of a change in your insurance coverage. This is very important because failure to
notify us of a change could result in an unexpected denial for your
services. Many
insurance companies require authorization for services prior to
delivery. Horizon Prosthetics will
request for authorizations on all services when applicable. Unfortunately, authorization is not a
guarantee of payment for the services.
You may
have a deductible and/or co-insurance payment according to your specific
insurance plan. Horizon Prosthetics
will do our best to determine your financial responsibility for services. Your estimated responsibility will be
discussed with you prior to delivery of services. For the best explanation of your prosthetic
insurance benefits call your insurance company directly or talk with your
