Please Note: It is important to bring your insurance cards at the time of your first appointment or anytime you change your insurance coverage or policy. A photo copy of your insurance card/s and a photo identification card, front and back will be made at the time of your appointment. It is your responsibility to notify Horizon Prosthetics of a change in your insurance coverage. This is very important because failure to notify us of a change could result in an unexpected denial for your services.
Many insurance companies require authorization for services prior to delivery. Horizon Prosthetics will request for authorizations on all services when applicable. Unfortunately, authorization is not a guarantee of payment for the services.
You may have a deductible and/or co-insurance payment according to your specific insurance plan. Horizon Prosthetics will do our best to determine your financial responsibility for services. Your estimated responsibility will be discussed with you prior to delivery of services. For the best explanation of your prosthetic insurance benefits refer to your benefit booklet, call your insurance company directly or talk with your Human Resource Administrator at work.
